The undersigned parent agrees that the child registered above is physically sound and wishes to participate on the Nomahegan Swim and/or Dive Team. Furthermore, the undersigned agrees to realize and discharge the swim club, its' officers and employees with respect to any injuries, damages or liabilities caused by their own negligence while participating in athletic competition. The undersigned parent also authorizes the club to post his/her child's name on the club's website in the event that the child achieves a swim team record.
